Forms for Students
New students and returning students must complete the Enrollment Application. This application is due August 21st.
Once enrollment or re-enrolled in the program has been confirmed, you will complete the 2026 - 2027 Student Enrollment Packet. Parents/guardians must submit this form prior to the beginning of Fall classes, which begin September 14th.
Additionally, we ask each CSA student to review and sign the Student Agreement. This outlines program expectations and encourages students to reflect on their individual goals for the year. Paper copies will be provided to students. You may also print the agreement and bring it to your instructor on the first day of class.

Frequently Asked Questions for CSA Families
Q: Where are CSA classes held, and where do I park?
A: We recommend families drive down Hill Ave (behind the church) and park in the CSUMS parking lot. You may then enter the breezeway through the entrance with a beige awning, walk straight through, then follow the signs for Community School of the Arts. You will turn left, enter your 5-digit door code (make sure to memorize this!), and head to your class. We ask that parents accompany students into the building.
Q: Can I drop my student off and return?
A: Unless a student is driving themselves to lessons, we ask that parents physically accompany their student into the building and check in to ensure safety. Parents/family members may leave and return, but must return prior to the end of class and must enter the building to check out their student.
Q: What do I do if my student has a scheduling conflict?
A: We ask that families plan ahead to prioritize class attendance. If a conflict is unavoidable, however, please communicate directly with your instructor as early as possible. Frequent unexcused absences may result in dismissal from the program.
Q: Who should I talk to if my student needs a specific accommodation or is experiencing a unique challenge?
We intend to set up every student for success and provide appropriate accommodations for students with disabilities. We also support students experiencing significant stress and challenging family situations. We rely on a trauma-informed approach to supporting these students. Q: I know families who may be interested in CSA. How do I connect them to the program?
A: If you know families who are interested in enrolling in CSA, please direct them to our website! We are especially interested in recruiting students who experience economic barriers to accessing arts instruction and those with a particular interest in music and/or visual arts.
Q: My student has been accepted into the program, now what?
A: Once your student has been accepted into the program, we will follow up to confirm enrollment in specific programs. We cannot necessarily guarantee that students will be able to take their 1st choice, but will do everything we can to match students with their preferences. Please review the CSA calendar, plan ahead to avoid conflicts (including recitals and special events), and complete the student enrollment packet.
Q: Who should I communicate with for last minute emergencies or questions?
A: For last minute emergencies or cancellations, please reach out directly to your instructor. You may also reach out to our staff if you need additional support.
Q: Where should I go during my child's lessons?
A: Parents and siblings are welcome to wait in the hallway during lesson times. Please obtain permission from staff before using any rooms since these are often reserved. CSA students and family members are not permitted to use the gym or playground before 6pm. Please read and follow any signs that are posted. There are many groups that use these spaces. Thank you in advance for respecting these rules so that we can respectfully share these spaces.
Q: Can I watch my student's lessons or sit in on their classes?
A: Unless assistance is required from a parent for a specific reason (e.g. to provide needed assistance to a student with a documented disability), we ask that parents do not sit in on lessons.
Q: How can I be supportive of my CSA student?
A: Encourage your student to practice, prepare, show up on time, and be curious. Cheer on your student in a recital or exhibition. Your involvement and encouragement makes all the difference.
